[18F]AlF-RESCA-IL2 is a radiopharmaceutical positron emission tomography (PET) imaging agent designed to non-invasively visualize and quantify the presence of activated T-lymphocytes within the body. It consists of the cytokine Interleukin-2 (IL-2) conjugated to an aluminum fluoride-18 ([18F]AlF) complex via a restrained complexing agent (RESCA). The tracer specifically targets the Interleukin-2 receptor alpha subunit (IL-2Rα), also known as CD25, which is highly upregulated on the surface of activated T-cells. Developed primarily to monitor the immune landscape in the tumor microenvironment, [18F]AlF-RESCA-IL2 allows clinicians to assess the infiltration and activation of tumor-infiltrating lymphocytes (TILs) in patients undergoing cancer immunotherapy, such as immune checkpoint inhibition. This functional imaging approach aims to provide a predictive biomarker for treatment response and to help distinguish between pseudo-progression and true tumor growth.
